Your First Visit
We understand that you or your loved one may be apprehensive about the first visit. We want to make sure that your first visit is as safe and comfortable as possible.
Our team of compassionate therapists will be able to assist you the moment you walk into our office. We will be happy to contact your insurance company to verify your mental health benefits, as this process can sometimes be overwhelming. Please make sure to have your insurance card available at the first appointment.
On your first visit, you will be greeted and given intake forms to fill out. For your convenience, we have provided the intake forms on our website. Completing the forms before your appointment will allow for more time to discuss your concerns with your therapist. If the new patient is a minor child (ages 17 or younger) the parent/guardian may be asked to provide a thorough history.
There are times when we may ask for other useful information from other professionals (schools, primary care physician, etc). If this applies, authorization from the client (parent/guardian if minor) is necessary in order to protect your confidentiality.
Depending upon your insurance coverage, co-pay, and co-insurance levels, we may require a credit card on file.
If you are planning to use insurance to help cover your therapeutic sessions, please fill out our HIPAA form to allow us to bill your insurance.
